Since 1922, Kittson Healthcare has proudly provided continuous service to the citizens of Kittson County and surrounding areas. Today, we are a 12-bed acute care federally designated Critical Access Hospital and a 50-bed long-term care facility, which also includes a home healthcare agency, an 8-bed assisted living facility, and ambulance services. Our campus includes a medical clinic in Hallock and a satellite clinic in Karlstad, MN. Job Summary:
The Home Health Aide provides compassionate, person-centered care and support to clients in their homes or assisted living settings. This role assists with activities of daily living such personal care nursing services, housekeeping, laundry, mobility, and/or meal preparation assistance that are designed to maintain the client’s physical and emotional well-being. In this role you will be expected to ensure a safe, clean, and comfortable environment. The aide observes and reports changes in the client’s condition to the appropriate healthcare team members, helping to maintain the individual’s dignity and independence as much as possible.
Training will be offered on the job.
Primary Responsibilities Include, but not limited to:
- Provide compassionate, person-centered personal care to clients, including assistance with bathing, grooming, dressing, toileting, oral hygiene, ambulation, and prescribed exercises under Registered Nurse (RN) guidance.
- Support clients in achieving maximum independence by offering choices and promoting self-help skills.
- Observe and report changes in clients’ physical, emotional, or cognitive condition to the supervising nurse or appropriate team member.
- Medication administration, assistance, and/or reminders with self-administration of medications as delegated by RN.
- Prepare and serve meals, assist with feeding, and follow special dietary instructions as needed.
- Perform health monitoring such as taking temperature, blood pressure, pulse, and respiration.
- Complete light housekeeping tasks to ensure a safe and clean environment, including laundry, waste disposal, sanitation, and maintaining supply levels.
- Plan and lead recreational and social activities that reflect tenant interests, promote engagement, and support emotional well-being.
- Communicate effectively and respectfully with tenants, families, and team members, demonstrating strong interpersonal skills and professionalism.
- Follow all policies related to client rights, privacy, and abuse prevention, including the Home Care Bill of Rights and Vulnerable Adults Act.
- Actively participate in safety procedures, emergency response, and injury reporting.
- Maintain accurate, timely documentation and submit required records according to facility policy.
- Attend required in-services, training(s), and staff meetings to maintain competency and meet annual education requirements.
- Transport or assist clients with transportation to appointments or outings as assigned.
- Use proper body mechanics and safe work practices to protect self and others during lifting or transferring.
- Maintain a flexible, dependable schedule and communicate promptly regarding absences or schedule changes.
Job Requirements:
Knowledge, Skills, and Abilities:
- Proficiently be able to read, write, speak, and understand the English language
- Ability to read, write, speak, and understand the English language to follow instructions, complete documentation, and effectively communicate with clients, families, and staff
- Ability to read and comprehend simple instructions, correspondence, and memos; write basic client documentation and communicate in both one-on-one and small group settings
- Proficiency in using specialized medical terminology and understanding basic healthcare practices and procedures
- Strong interpersonal skills with the ability to work collaboratively as part of a care team including RNs and other aides
- Demonstrates maturity, emotional stability, and the ability to remain calm, kind, and composed in difficult or unexpected situations
- Able to prioritize and organize work effectively and efficiently while remaining flexible and adaptable to changing client needs and environments
- Compassionate, ethical, and tactful in delivering person-centered care that promotes client independence and dignity
- Ability to successfully complete Health Care/Assisted Living Aide training
- Comfortable using computers for electronic charting and completing required online training or in-services
- Must maintain a reliable attendance record and demonstrate punctuality and accountability in fulfilling scheduled shifts
Qualifications:
- High school diploma or equivalent required; Nursing Assistant Certification, Associate’s Degree in Gerontology, Medial Assisting Health Sciences, or a related field preferred.
- Valid MN Driver’s License
- Must pass MN background check and fingerprinting
- Complete TB testing within the first 3 weeks of employment
Job Schedule/Location(s):
- Location(s) | Kittson County
- Part-Time | 8:00am – 2:00pm OR 10:00am – 5:00pm (shifts vary depending on client and department needs)
- Every other weekend and holiday rotation required
